Tummy Tuck Recovery Week by Week

By Op. Dr. Göktuğ Esmer · Updated: 26 September 2026 · 6 min read

Tummy tuck recovery moves through three broad phases: the first 2 weeks of walking slightly bent forward, managing drains and resting; weeks 2 to 6, when you gradually straighten up and return to everyday life; and from week 6 to 8, when exercise can restart. The final result, once swelling has fully settled and the scar has matured, appears at 6 to 12 months. The timeline below is a general guide; your own course depends on the extent of your surgery and your surgeon's instructions.

Why tummy tuck recovery is different

A tummy tuck (abdominoplasty) is more than removing loose skin and fat. In most patients the separated abdominal muscles (diastasis recti) are stitched back together along the midline. This muscle repair is the most valuable part of the operation, and it is also the part that asks the most patience during recovery. Every time you cough, stand up or twist, the abdominal wall is working, so it needs protecting in the early weeks. The hip-to-hip incision also has a long line to heal. For these two reasons a tummy tuck recovers more slowly than liposuction, but delivers a far more fundamental change.

The first week: bent posture, drains and walking

Why do I walk bent forward?

Because the abdominal skin is pulled down and stitched under tension, standing bolt upright in the first days strains the suture line. Walking with a slight bend at the hips and waist takes tension off the incision and helps keep the scar thin. This posture usually lasts 7 to 14 days and you straighten a little more each day. Do not force yourself upright; your body opens up at its own pace.

Drains

Thin silicone drains are placed to remove fluid that collects under the skin. The daily output is recorded, and once it drops below a set level, usually between day 5 and day 10, the drains are removed. Removal takes a few seconds and is more strange than painful. Some techniques avoid drains altogether; that decision rests with your surgeon.

Why walking is non-negotiable

From the evening of surgery you will be asked to take short, supported walks. The reason is to reduce the risk of blood clots in the leg veins (deep vein thrombosis). The rule is not "bed rest" but "frequent short walks with rest in between". Walking also gets the bowels moving and keeps the lungs expanded.

Sleeping position and daily life

For the first 2 to 3 weeks the most comfortable position is on your back, with pillows under your knees and your head raised, the so-called beach-chair position. It takes the tension off the abdominal skin. A recliner or a stack of pillows works well. Side sleeping is usually comfortable after week 2 or 3, and sleeping on your stomach after week 6. To get out of bed, roll onto your side first and push up with your arms rather than sitting up using your abdominal muscles.

Showering is normally allowed once the drains are out. Baths, the sea and swimming pools must wait until the incision is completely healed, which means at least 4 to 6 weeks. Lifting, including small children, is limited to around 3 to 5 kg for the first 4 to 6 weeks. Smoking is the single biggest factor that impairs wound healing and must be stopped before and after surgery.

Garment and swelling

As after liposuction, the compression garment controls swelling and helps the skin settle into its new position. It is usually worn all day for the first 4 to 6 weeks and then for shorter periods as your surgeon advises. Swelling fluctuates in the early weeks; it is normal for it to be worse in the evening and better in the morning. The area just above the incision and the pubic region are the last to settle and can stay firm and puffy for 3 to 6 months. Numbness of the lower abdominal skin is common too; sensation usually returns gradually over the following months.

Scar care: when does the scar fade?

The tummy tuck scar is planned to sit below the bikini or underwear line. Its appearance follows a predictable course:

  1. First 6 weeks: Pink to red, slightly raised, sensitive.
  2. Months 2–6: The colour can deepen and itching is common; this is when the scar looks its most obvious.
  3. Months 6–12: The scar starts to fade, flatten and soften.
  4. Months 12–18: The scar matures and reaches its final appearance.

The basics of scar care are simple: use silicone gel or silicone sheets once the incision has closed, protect the scar from the sun for at least a year (cover it or use a high-factor sunscreen) and avoid stiff belts or elasticated waistbands that rub along the line. If you are prone to problem scarring (keloid or hypertrophic scars), tell your surgeon before the operation.

Returning to work and exercise

  • Desk work: Usually from week 2, building up to full days gradually.
  • Physical work: 6 weeks or more.
  • Walking: From day one, increasing the duration daily.
  • Light cardio (stationary bike, elliptical): Weeks 4 to 6, with your surgeon's approval.
  • Weights, core exercises, running, swimming: After weeks 6 to 8, once the muscle repair has consolidated; core work is the last thing to reintroduce.

When you do return to the gym, start with around half your previous loads and increase weekly. It is both safer and more sustainable. If a movement creates pulling or pain along the incision, it is still too early for that movement.

When will I see the final result?

Your abdomen looks flat from the moment you leave the operating table, but swelling, firmness and scar changes mean the true result cannot be judged for at least 6 months. By 12 months the skin has fully settled, the scar has faded and the waistline is well defined. As long as your weight stays stable, the result is long-lasting; a future pregnancy or a significant weight change can affect it. Patients who want to address post-pregnancy changes to the breasts and abdomen in one operation may wish to consider a mommy makeover.

For international patients

Patients who travel to Istanbul for a tummy tuck usually stay in the city until the drains are out and the first check-ups are complete. On the flight home, wear your garment, choose an aisle seat and walk for a few minutes every hour. Later follow-ups continue with photos and video calls; the details are on our international patients page.

Frequently Asked Questions

How long do I walk bent forward after a tummy tuck?

Usually 7 to 14 days. As the tension along the incision eases you straighten a little more each day, and by the end of week 2 most patients walk almost upright. Forcing yourself straight can strain the suture line.

When are the drains removed?

Once the daily output falls below a set level, typically between day 5 and day 10. Removal takes a few seconds. Some techniques do not use drains at all; that is your surgeon's decision.

How should I sleep after a tummy tuck?

For the first 2 to 3 weeks, on your back with pillows under your knees and your head raised (the beach-chair position) is the most comfortable and safest option. Side sleeping is usually fine from week 2 or 3 and stomach sleeping after week 6.

When can I exercise again?

Walking starts on day one. Light cardio can begin around weeks 4 to 6, and weights and core exercises after weeks 6 to 8, once the muscle repair has consolidated and your surgeon has given approval.

Will the tummy tuck scar disappear?

The scar never vanishes completely, but it fades and flattens significantly over time. It looks most obvious between months 2 and 6 and matures over 12 to 18 months. Silicone gel and sun protection improve its final appearance.

When will I see the final result?

Most of the result is visible by month 3; the final result, once swelling has fully settled and the scar has matured, is clear at 6 to 12 months.
