What is a BBL and fat transfer?
Fat transfer, or lipofilling, moves living fat cells from an area with surplus to an area that needs volume. Because the material is your own tissue, there is no risk of allergy or rejection, and fat that survives becomes a living part of its new location.
A BBL is fat transfer applied to the buttocks. Three things determine the result: liposuction that sculpts the waist and surrounding areas, harvesting and purifying the fat without damaging it, and injecting it into the correct layer with an even distribution. Unlike buttock implants, nothing foreign is left in the body, but the achievable increase is limited by how much fat can be harvested.
Who is a candidate?
A good candidate has enough fat to harvest from the abdomen, flanks, back or thighs, a stable weight, good general health and realistic expectations. Common reasons for seeking a BBL include:
- Flat or small buttocks, or a "square" shape
- A poorly defined waist-to-hip ratio
- Loss of volume or sagging after weight loss or with age
- A wish to fill hip dips
- Looking for an alternative to buttock implants
Patients with a very high body mass index are advised to lose weight first for safety reasons. Active smoking, uncontrolled diabetes, clotting disorders and a recent history of thrombosis are assessed carefully during planning. Where there is significant loose skin, fat transfer alone may not be enough and liposuction may be combined with skin-tightening procedures.
Techniques and options
Harvesting the fat (liposuction)
Fat is removed from the abdomen, flanks, back and inner thighs with fine cannulas at low vacuum pressure to protect the cells. This stage also sculpts the waistline: a narrower waist visually amplifies the increase in the buttocks. The liposuction method (traditional, VASER or power-assisted) is chosen according to your tissue.
Purifying the fat
The harvested fat is separated from blood, fluid and damaged cells by settling, washing or centrifugation. The aim is to maximise the proportion of viable fat cells so that as much of the injected volume as possible survives.
Safe injection: subcutaneous only
The single most important safety rule in BBL is that fat is placed only in the subcutaneous fat layer, never into the muscle. Intramuscular injection carries the risk of fat entering large veins (fat embolism). Dr. Esmer therefore works with thick, blunt cannulas at low pressure, keeping the cannula tip superficial at all times.
Ultrasound-guided BBL
In line with international society recommendations, the position of the cannula can be tracked in real time with ultrasound during injection. This confirms that the cannula stays in the subcutaneous layer and never passes beneath the muscle fascia.
Fat survival and longevity
Part of the injected fat is reabsorbed in the first weeks; what remains at three months is considered permanent. The survival rate depends on your tissue, smoking, nutrition, adherence to sitting rules and technique.
Fat transfer to other areas
Hip dips can be filled in the same session; small volumes can rejuvenate the face and hands, and suitable patients may plan fat transfer to the breast.
First consultation and planning
The process can start with an online assessment: front, side and back photographs, height and weight, and examples of the look you have in mind allow Dr. Esmer to judge whether there is enough fat and to outline a likely plan.
At the in-person examination, each potential donor area is assessed, skin quality and elasticity are checked, and your waist-to-hip ratio and buttock shape (A, V, square or round) are identified. The aim is to choose a proportion that suits your frame; realistic outcomes are discussed rather than exaggerated social-media examples.
Pre-operative tests include a blood count, clotting tests, biochemistry and an anaesthetic review. Thrombosis risk factors such as the contraceptive pill, family history and immobility are checked and additional precautions planned where needed. The injection technique, expected fat survival, sitting rules and recovery timeline are explained in detail.
Before surgery
Fat survival and the prevention of complications depend heavily on preparation:
- Smoking and nicotine: stop at least four weeks before and for four weeks after surgery, including vaping; nicotine sharply reduces fat cell survival.
- Blood thinners and aspirin: aspirin, ibuprofen, vitamin E, omega-3 and herbal supplements are paused around ten days beforehand; prescription anticoagulants are adjusted only with medical approval.
- Contraceptive pill: a four-week break before surgery may be advised because of thrombosis risk.
- Alcohol: none in the final week.
- Weight: do not diet before surgery; your fat reserves are the material we work with.
- Fasting: nothing to eat or drink from eight hours before surgery.
- What to bring: passport or ID, medication list, loose comfortable clothing and slippers.
- Garment and BBL pillow: the compression garment and special seating pillow are provided by the team.
The operation day
You are admitted in the morning. With you standing, Dr. Esmer marks the donor areas and the injection plan on the skin. After the anaesthetic review, surgery is performed under general anaesthesia; throughout the operation, pneumatic compression devices on the legs guard against clots and your body temperature is maintained.
Depending on the number of donor areas and the volume transferred, the operation usually takes 2–4 hours: liposuction first, then purification and injection. A compression garment is fitted at the end. Drains are not normally needed for liposuction areas; some fluid leakage from the small cannula entry points in the first 24–48 hours is normal.
You stay one night in hospital with pain control, fluids and early walking, and are discharged after a check the next morning.
Recovery and aftercare
In the first days you can expect bruising, swelling and a muscle-ache-like soreness in the liposuction areas, with fullness and tightness in the buttocks. The most important rule is to avoid pressure on the transferred fat cells.
- Sitting rules: no direct sitting on the buttocks for the first 2–3 weeks. If you must sit, place the BBL pillow under your thighs so the buttocks hang free, and keep it brief. From week three, sit gradually with the pillow; avoid prolonged sitting until 6–8 weeks.
- Sleeping position: on your front or side for the first three weeks; never on your back.
- Garment: worn day and night for six weeks over the liposuction areas, making sure it does not compress the buttocks.
- Showering: usually from 48 hours; keep the cannula sites dry between showers.
- Activity: walking starts on the day of surgery. Light cardio at four weeks, and glute-loading weight training after eight weeks.
- Lymphatic drainage massage: only on the liposuction areas and with Dr. Esmer's approval; the buttocks are never massaged.
- Nutrition: no calorie restriction after surgery; adequate protein and fluids support fat survival.
- Scar care: cannula scars are a few millimetres long and fade with silicone gel and sun protection.
Most swelling settles by six weeks; the permanent volume is clear at three months and the final shape at six months.
Risks and how they are minimised
Performed with the right technique, a BBL is a safe operation, but it is important to understand its risks and the precautions taken.
- Fat embolism: rare but the most serious risk, caused by fat entering the muscle and large veins. Subcutaneous-only injection, blunt thick cannulas, low pressure and ultrasound guidance keep this risk to a minimum.
- Deep vein thrombosis and pulmonary embolism: reduced by compression devices, early walking, anticoagulant injections where indicated and adjusting risk factors beforehand.
- Fat necrosis and oil cysts: hardening of fat that fails to survive; limited by not overfilling and by distributing fat in small parcels.
- Infection and seroma: prevented by sterile technique, prophylactic antibiotics and the compression garment.
- Asymmetry and contour irregularity: possible in liposuction areas or the buttocks; reduced by careful marking and balanced injection.
- Volume loss: some reabsorption is natural; smoking and early sitting increase it.
- Sensory changes: usually temporary.
Safe volume limits per session, a fully equipped hospital and an experienced anaesthetic team are decisive in managing these risks.
For international patients
For patients travelling from the UK, Europe and the Middle East, planning is largely completed through the online assessment; in Istanbul you have your examination, tests and final planning, and surgery usually takes place within a day or two.
For a BBL we recommend staying 7–10 days in Istanbul, covering the first checks and a final review before you fly. The sitting rules apply on the flight: use your BBL pillow, choose a wider seat where possible, stand and walk at regular intervals and wear compression stockings. Dr. Esmer confirms when you are fit to fly.
The coordination team arranges airport transfers, accommodation near the hospital and an interpreter if needed. Once home, follow-up continues through WhatsApp photo updates and scheduled video calls. See our international patients page for details.
How the price is determined
Searches for BBL Turkey cost or fat transfer prices are common, but under Turkish regulations medical services cannot advertise prices and there is no price list on this website. A personalised quotation is provided after evaluation.
The main factors that influence the fee are:
- The number of liposuction areas and the volume of fat harvested
- The liposuction technology used and ultrasound guidance
- Operating time, anaesthesia and hospital stay
- Additional transfer areas such as hip dips, breasts or face
- Pre-operative tests, garment, BBL pillow and medication
- Accommodation and transfer packages for international patients
A transparent, itemised quote follows your online assessment.

Frequently Asked Questions – BBL in Turkey: Brazilian Butt Lift and Fat Transfer
How much of the fat survives after a BBL?
Part of the injected fat is reabsorbed in the first weeks, and the volume remaining at three months is considered permanent. Not smoking, following the sitting rules and keeping a stable weight all improve survival.
When can I sit after a BBL?
No direct sitting on the buttocks for the first two to three weeks. After that you sit gradually with a BBL pillow under the thighs, and prolonged sitting is avoided until six to eight weeks.
Why is fat injected only under the skin?
Injecting into the muscle risks fat entering large veins, known as fat embolism. Subcutaneous-only injection with blunt cannulas under ultrasound guidance is the current safety standard that minimises this risk.
I am very slim. Can I still have a BBL?
When there is little fat to harvest, the achievable volume is limited. Donor areas are assessed at the examination; if they are insufficient, realistic goals or alternative options are discussed.
What happens if I gain weight after a BBL?
Surviving fat cells are living tissue and respond to weight change like the rest of your body. Significant weight gain or loss can alter both the buttock volume and the liposuction result.
How soon can I return to work?
Desk-based work is usually possible within 10–14 days using a BBL pillow. Physically demanding jobs should wait four to six weeks.
How do I manage the flight home?
After 7–10 days in Istanbul, use your BBL pillow on the flight, stand and walk regularly and wear compression stockings. Dr. Esmer confirms you are fit to fly at the final check.
Are buttock implants better than a BBL?
Implants provide volume when there is no fat to harvest but involve a foreign body and different risks. A BBL uses your own tissue and sculpts the waist in the same session.
