Most people notice the same pattern after a BBL (Brazilian butt lift, or fat transfer to the buttocks): the first two to three weeks are the most restrictive, with no direct sitting, sleeping on your front or side and a compression garment worn day and night. Swelling then settles gradually over the following weeks, and the final shape typically becomes clear somewhere between three and six months. Exact dates differ from one surgeon to the next, so treat every timeline below as typical rather than fixed.
This guide walks through recovery week by week for patients planning a BBL in Istanbul. It draws on the aftercare rules on our procedure page and on published guidance from the plastic surgery societies ASPS and ISAPS and from Cleveland Clinic. Where those sources disagree, we say so, because that is exactly the kind of detail your own surgeon will settle for you.
BBL recovery at a glance
| Period | What you can typically expect | Main rules |
|---|---|---|
| Days 1–3 | Bruising, swelling, soreness in the liposuction areas, a feeling of fullness in the buttocks | Short walks, garment on, no sitting on the buttocks, sleep on your front or side |
| Week 1 | Swelling and bruising are most visible, then begin to ease; tiredness is common | Rest, short walks, dressing changes and checks, pillow only when sitting is unavoidable |
| Weeks 2–3 | Noticeably more comfortable; some people return to desk-based work | Still no direct sitting; garment continues; surgeon may approve lymphatic massage on the liposuction areas |
| Weeks 4–6 | Swelling eases further; light cardio can begin | Gradual sitting with a pillow; garment tapers around week 6 |
| Weeks 6–12 | Shape looks more natural as swelling settles | Normal sitting returns; exercise that loads the buttocks waits until after week 8 |
| Months 3–6 | The volume that remains becomes clear and the contour keeps refining | No restrictions; keep your weight stable |
Why BBL recovery is built around protecting the fat
In a BBL, fat is taken from areas such as the waist, abdomen or back by liposuction, purified and injected beneath the skin of the buttocks. The transferred cells have to establish a new blood supply, and sustained pressure during that period can reduce how much of the fat survives. ASPS describes this as pressure necrosis, where pressure damages the fat cells, which is why sitting and sleeping positions matter so much in the first weeks.
Some of the transferred fat is reabsorbed in every BBL. In an ASPS article a surgeon puts the average amount of fat that survives at about 60%, though this varies between individuals and techniques. Your aftercare cannot change the surgeon's technique, but it is one of the few factors you control yourself.
Days 1–3: hospital and the first nights
Surgery takes place in a fully equipped hospital with anaesthesia and intensive care backup, and you normally stay one night, as described on our procedure page. Walking starts on the day of surgery, because early movement supports circulation and helps lower clot risk.
Cleveland Clinic notes that the first several days are often the most painful. Expect bruising, swelling, a muscle-ache-like soreness in the liposuction areas and a sense of fullness or tightness in the buttocks. A little fluid leaking from the small cannula entry points in the first 24 to 48 hours is normal. Pain is managed with the medication your surgeon prescribes.
- Sleeping: on your stomach or side. ASPS advises stomach sleeping from the first night until your surgeon says otherwise.
- Showering: usually possible from about 48 hours, keeping the entry points dry between showers.
- Company: arrange for a relative or friend to be with you, at least for the first 24 hours.
- Checks: dressings are changed and your recovery is checked daily in Istanbul.
Week 1: rest, short walks and no sitting on the graft
ASPS describes the first four to five days as mostly staying at home with little activity beyond short walks. For an international patient that means resting at your accommodation, walking for a few minutes at regular intervals and keeping the garment on as instructed. ASPS describes wearing it around the clock, except when showering, for the first month.
If you must sit, the usual solution is a BBL pillow, a firm triangle-shaped cushion that moves the pressure onto the backs of your thighs. Both ISAPS and ASPS treat using the toilet as an acceptable exception. Standing to eat, reading or working on your phone while lying on your front are practical alternatives. Our guide on when you can sit after a BBL covers pillow technique, sleeping and driving in more detail.
Mobility and swelling in week 1
ISAPS notes that walking is greatly encouraged after the first full week because it improves circulation and healing. Swelling and bruising usually look most pronounced in the first days and then ease gradually.
Weeks 2–3: feeling better, rules still apply
Cleveland Clinic says pain usually decreases after one to two weeks. This is the point where many patients feel they can do more than they should, so it helps to know the rules that remain:
- Sitting: our procedure page advises no direct sitting on the buttocks for the first two to three weeks. Published guidance ranges from 10 to 14 days (ISAPS) to at least two weeks (Cleveland Clinic).
- Work: return to work is commonly quoted at between about 7 and 14 days, depending on the job. Desk-based work is possible using a pillow or a standing setup.
- Lymphatic massage: if your surgeon approves, it is applied only to the liposuction areas and never to the grafted buttocks.
- Driving: wait for your surgeon's approval and avoid driving while on prescription painkillers.
Weeks 4–6: swelling eases, activity widens
Both ISAPS and Cleveland Clinic advise avoiding heavy exercise and strenuous activity for at least about a month. Our procedure page lists light cardio from around week 4, and sitting with the pillow becomes gradually longer. Avoid prolonged sitting until about weeks 6 to 8; ASPS describes the first six weeks as the period in which you need to be careful about prolonged pressure.
How long do you wear the garment?
Garment advice varies. ASPS quotes wearing it around the clock for the first month and at least 12 hours a day in the second, while ISAPS mentions up to eight weeks. Our procedure page describes about six weeks over the liposuction areas, designed so that it does not squeeze the buttocks. Your surgeon will set the exact schedule, and our guide to compression garment duration after liposuction explains what the garment does for the donor areas.
Weeks 6–12: back to normal sitting, building up exercise
By this stage many patients are sitting normally again and managing a full working day, provided their surgeon has cleared them. Exercise that loads the buttock muscles, such as squats, lunges and hip thrusts, usually waits until after week 8 and until your surgeon agrees.
The buttocks may look slightly smaller as swelling goes down. That change is partly swelling resolving and partly the natural reabsorption of some fat, and in the early months it is hard to tell the two apart, which is one reason photo comparisons at set intervals are useful. ASPS notes that around month three the acute risk of losing fat from the area subsides.
Months 3–6: when the result settles
ISAPS points out that early results are masked by swelling and that the final result can take up to six months or longer to appear. ASPS describes months three to six as the time when the shape continues to refine. Cleveland Clinic gives a shorter window, saying the body should recover fully between two and three months and listing follow-up visits at 1 to 2 weeks, 4 to 6 weeks and 3 months.
Fat cells that survive behave like fat elsewhere in your body, so significant weight gain or loss can change the result. Staying close to your pre-surgery weight is the simplest way to protect it. Because the liposuction areas swell and settle on their own schedule, you may also find our guide to the liposuction swelling timeline useful.
Why published timelines differ
The table below compares what the three sources we used say. The differences are real, and they reflect variations in technique, the volume of fat transferred and individual healing.
| Topic | ASPS | ISAPS | Cleveland Clinic |
|---|---|---|---|
| Sitting | Be careful about prolonged pressure for the first 6 weeks; keep sitting within about 10 minutes | No sitting for 10–14 days, except the toilet | No sitting or lying on the buttocks for at least 2 weeks; about 8 weeks until sitting without a pillow |
| Garment | Around the clock for the first month, at least 12 hours a day in the second | Up to 8 weeks | May be needed over the liposuction areas |
| Back to work | Days 7–10 | Not specified | Days 10–14 |
| Final result | Refines over months 3–6 | Up to 6 months or longer | Full recovery in 2–3 months |
This is why a single generic calendar should never override your own surgeon's instructions. Follow the plan you are given in writing, and ask when anything is unclear.
Warning signs and risks
A BBL is major surgery with real risks. Cleveland Clinic lists injury to blood vessels or nerves, fat embolism, infection, bruising, skin discoloration, sensory changes and poor wound healing. Our procedure page adds fat necrosis, oil cysts, asymmetry and some loss of volume, and explains that fat is placed only in the layer beneath the skin, with ultrasound guidance, to reduce the most serious of these risks.
Contact your surgical team promptly if you develop a fever, redness that spreads, pain that your medication does not control, unusual discharge, or swelling and pain in one calf. Chest pain or difficulty breathing needs emergency medical care straight away, wherever you are.
Planning your Istanbul stay and the flight home
Our procedure page recommends staying about 7 to 10 days in Istanbul, which covers the first checks and a final review before you fly. Your surgeon confirms the exact date individually. Details on travel planning are on our international patients page.
- Take your BBL pillow in your hand luggage and use it for the whole flight.
- Wear compression stockings, stand and walk regularly, and choose a wider seat if possible.
- After you return home, follow-up continues by WhatsApp photos and scheduled video calls, and your coordinator can be reached at any hour.
If you are still comparing surgeons and countries, our checklist on how to choose a surgeon abroad is a useful starting point.
What helps recovery go smoothly
- Stop nicotine early. Our procedure page asks patients to stop at least four weeks before surgery and stay off it for four weeks afterwards. See our article on smoking, alcohol and blood thinners before surgery.
- Practise sleeping on your front for a few nights before you travel.
- Do not diet after surgery. Adequate protein and fluids support healing.
- Think carefully about combining procedures. Combining a BBL with other operations adds to the recovery load, so read about combining a mommy makeover with a BBL before deciding.
- Understand the liposuction stage. The donor areas shape much of the visible result, and the technique is explained on our liposuction page.

Frequently Asked Questions
How long does BBL recovery take?
Most people feel noticeably more comfortable after 2 to 3 weeks, and desk-based work is often possible within about 1 to 2 weeks using a pillow. Swelling and shape take longer to settle: the contour typically keeps refining until 3 to 6 months. Timelines vary between surgeons and individuals.
When can I sit normally after a BBL?
Direct sitting on the buttocks is avoided for about 2 to 3 weeks on our procedure page, while published sources range from 10 to 14 days to at least 2 weeks. Sitting then returns gradually with a BBL pillow, and prolonged sitting is generally avoided until about 6 to 8 weeks. Your surgeon confirms the schedule.
How long do I wear the compression garment after a BBL?
Our procedure page describes about 6 weeks over the liposuction areas, worn so that it does not squeeze the buttocks. Other sources quote around the clock for the first month and at least 12 hours a day in the second, or up to 8 weeks. Follow the schedule your surgeon gives you.
When can I exercise after a BBL?
Short walks start from the day of surgery. Heavy exercise is usually avoided for at least a month, light cardio typically begins around week 4, and exercises that load the buttocks, such as squats and hip thrusts, usually wait until after week 8 and your surgeon's approval.
When will I see my final BBL result?
Early results are masked by swelling. The volume that remains is generally clearer at about 3 months, and the shape can keep refining for up to 6 months or longer. Significant weight gain or loss can change the result, so keeping your weight stable matters.
When can I fly home after a BBL?
Our procedure page recommends staying about 7 to 10 days in Istanbul, covering your first checks and a final review before you fly. Your surgeon confirms the exact date. On the flight, use your BBL pillow, wear compression stockings and stand and walk at regular intervals.
